Profile of NGO (SAMPLE)
NGO (SAMPLE) has over 5 years’ experience in community development work and provision of logistics within Ebonyi and Enugu States (this is a sample) and has implemented extensive projects in health, human rights, environment and water and building capacity of community institutions and local government participants with a focus on participatory management of resources. NGO (SAMPLE) is currently providing logistics support services for Malaria Action program for State MAPS project supported by USAID. In this status, NGO (SAMPLE) has the mandate to provide logistics support for Malaria prevention, case management and diagnosis. NGO (SAMPLE) is a member of several networks: CISHAN, National TB Network, ACOMIN, CENGOS, SEGON. NGO (SAMPLE) is currently concluding a community social mobilization programme in 192 communities of 3 LGAs of Ebonyi State funded Global Fund/SFH on Malaria intervention.

NGO (SAMPLE) Organizational Structure.
Our organization comprises of the board of trustees, the Executive Director, Program manager, Technical consultant, Finance, Monitoring and Evaluation, Database, Community mobilization and 10 logistics officers.
Our strategies include
Advocacy (Social and Legislative); Awareness Raising and Capacity Building; Research Documentation and Publication; Paralegal Services; Monitoring and Evaluation.

Operational approach; Staffing and team  Composition
For the sake of this project implementation, the management of NGO (SAMPLE) has 6 key highly qualified staff with vast knowledge in program management. NGO (SAMPLE) also has 10 project staff in charge of logistics and over 1500 network of community based peer educators in the state. The 6 key staff, whose abridged CVs are hereby attached are: i). Dr Agboeze Joseph – Technical consultant ii) Nancy Ifeyinwa Oko-Onya – Executive Director, iii). Ndubuisi Oji – Programme Manager, Onyinye Udenze Monitoring and Evaluation/Database Specialist iv) Emmanuel Okorie - Project Accountant v) Amarachukwu Agbaeze – Community Mobilization officer.
Methodology/Understanding of the program
In executing this all important assignment, NGO (SAMPLE) is going to deploy its participatory cluster-based approach in the doing the following

MAPPING
1.    Mapping by LGA and zone the contact details of PPMVs and Community pharmacists most of which their database we already have.
2.    Mapping of training venues and hotel accommodations for trainers.
3.    NGO (SAMPLE) will also do a mapping of available and qualified trainers within the Federal Teaching Hospital to add to our database.
4.    Mapping of reputable catering institutions or firms with the capacity to deliver meals to 1,200 participants with trainers.

SERVICE DELIVERY
Depending on the number of days required and number of LGAs to be covered the training will be divided into different events and clusters. Such as;
Event 1- Training of Community Pharmacist (CPs) on Pneumonia Case Management
Event 2- Training of Proprietary Patent Medicine Vendors on Pneumonia Case Management.

CLUSTER
A cluster will be made up of 25 to 30 participants and will be supported by 2 NGO (SAMPLE) logistics officers. The logistics officer will prepare the training venue for use for the day, administer and collect, pre and post-test evaluation, take attendance, ensure that all required training materials are provided (projector, screen, public address systems, flip chart markers, writing materials and name tags). They will also ensure tea and lunch is served and make payments where necessary.

A TRAINING EVENT
A training event is a training activity structured to happen simultaneously across different clusters on a particular topic or module within a period of time. For the sake of this project, an average of 1 training event of 5 clusters will happening at the same time across the chosen locations. For this training, NGO (SAMPLE) will be deploying 5 training events in 10 training clusters to be able to deliver training for 1,200 participants.

REPORTS
NGO (SAMPLE) monitoring and evaluation team will develop activity reports, training reports, analyse pre and post-test evaluation results and a training database of all trained participants with their full contact details. NGO (SAMPLE) will also develop an album of action pictures of the training event.

The finance unit will also prepare a comprehensive financial income and expenditures analysis of the training event.

Resource Persons/Trainers
NGO (SAMPLE) has a database of trainers made up of 6 medical doctors; 2 of which are paediatrics, 4 nurses, 2 laboratory scientist. 

A total of 30 NGO (SAMPLE) staff will be involved in this project; namely 1. 6 Program staff, 2. 10 Logistics staff, 3. 10 Trainers/Resource Persons and 4 Drivers.

Physically/Verifiable Offices: NGO (SAMPLE)’s project office is located at NO………., with four (4) room space, a mini conference room that can take about 30 persons.

Office Tools & Equipments: NGO (SAMPLE) has a well furnished office apartment with IT equipments (computers and accessories, printers’ internet routers, projectors and screen and power generating set.

Project Vehicles & Motorcycles: 1 Hilux project vehicle and a We have a project vehicle.

Quality of Project Implementation: All programs and projects implemented or managed by NGO (SAMPLE) are known for meeting and exceeding its set targets and objectives both in quality and quantity. The letters of recommendations and references from our supporters, donors and network organizations are attached herewith to proof this claim.
